You could also envision situations where a process is " exothermic ", but " endergonic ", that is work is done ON the system, but heat is release BY the system.

In metabolism, an endergonic process is anabolic, meaning that energy is stored; in many such anabolic processes energy is supplied by coupling the reaction to adenosine triphosphate ( ATP ) and consequently resulting in a high energy, negatively charged organic phosphate and positive adenosine diphosphate.
Where the metabolism of sugars and the metabolism of alcohol merges is when they are converted into an acetyl group and bound as a thioester with coenzyme A to form acetyl-CoA . The first step of ethanol metabolism involves its conversion into acetaldehyde, and this * costs * energy ( it's endergonic ).
